From Nicole Bensley, our ON BOCES Certification Officer (Office in Medina):
After people request the name change in TEACH, they need to send NYSED a copy of documentation to back up their request or their name will not be changed. They send a photocopy of the marriage license or documentation of a divorce, etc.
The documentation can be mailed to NYSED at 89 Washington Avenue, Room 5N, Albany, NY 12234 along with a brief letter to explain what the documentation is or the documentation can be emailed to email@example.com. I recommend email if someone can scan the materials and attach it to the email. The name change documentation is processed very quickly via email.
(The TRC can scan your documents for you.)
There is not a fee to change a name in TEACH unless a printed certificate is requested. If a printed certificate is desired, then the person would apply for a duplicate certificate in TEACH and pay $25 after they request a name change.
Professional development records before October 2014 will be available on the TRC transcripts.
PDP Premier user names are an individual's NFCSD email address and a password was sent to everyone in a hard copy notice in October.
